News in NIV remote monitoring: ERS presents its evidence-based recommendations

On Sunday 28 September, at its 2025 congress in Amsterdam, the European Respiratory Society presented its practical recommendations (1) for telemedicine in home mechanical ventilation (HMV). The number of patients with respiratory failure receiving ventilation at home, particularly non-invasive ventilation (NIV), is increasing and treatment methods are evolving.A group of 20 experts, including a patient representative and her caregiver, conducted systematic research in the MEDLINE, EMBASE, Cochrane and CINAHL databases. Based on this analysis of the literature, the group formulated the following clinical practice recommendations for the use of telemedicine in HMV:

Recommendation for long-term ventilation monitoring using telemedicine
Advanced algorithms and AI: drivers for the futureIn terms of quality, the authors note that, overall, the studies reviewed report a high level of satisfaction with telemedicine among patients and caregivers.According to them, the potential of telemedicine to improve access to home ventilation care faces local obstacles: connectivity issues, lack of reimbursement, regulatory gaps, and concerns among clinicians.Finally, the authors identify technological advances and the use of advanced data processing algorithms and artificial intelligence as ‘drivers for future research and the use of telemedicine’.
